This survey is being conducted by Abt Associates Inc. and the Education Development Center for the National Aeronautics and Space Administration (NASA) as part of its efforts to learn about the teachers who participate in the Summer of Innovation program. We estimate that it will take approximately 10 minutes to complete the survey.
Privacy and Participation
You will never be identified by name, and information from the evaluation will only be reported in the aggregate. All information that would permit identification of individual respondents will be held in confidence, will be used only by persons engaged in and for the purposes of the national evaluation, and will not be disclosed or released to others for any purpose except as required by law. We ask for your name below only for the purposes of linking your responses now to those from the baseline survey and any future surveys. Your data from these surveys will be linked with a study identification number. Your responses to this survey will be protected under the Privacy Act. There is minimal risk of breach of confidentiality, and we have put in place procedures to minimize this risk.
Your participation in the study is voluntary and nonparticipation will have no impact on you, your school, or your SoI awardee organization. You can refuse to answer any question and you can quit the survey at any time.
