The National Institute on Deafness and Other Communication Disorders (NIDCD), part of the National Institutes of Health, is conducting a survey to evaluate a campaign to increase awareness and prevent Noise-Induced Hearing Loss (NIHL). The NIDCD wants to learn what the public thinks about NIDCD educational materials to prevent NIHL.
We are asking you to participate in this survey since our records indicate that you placed an order for Noisy Planet materials, either online or by phone this past year. Completing the survey should take about 10 – 12 minutes. Please help the NIDCD by giving your opinion of the Noisy Planet materials that you ordered.
Your participation in this survey will help the NIDCD create an even more effective campaign to reach youth ages 8 to 12 and their parents. NIHL is a preventable health problem that affects an estimated 5.2 million children and adolescents.
