I am writing to ask for your help with an important survey being conducted for the U.S. Food and Drug Administration (FDA). This survey will help the FDA better understand how people think about communications related to the drugs, devices, and other medical products that it regulates.

Your household has been randomly selected to participate in this survey. This letter is addressed to “Resident” because all households, including yours, were selected based on the address listed. The FDA does not know for sure who lives at the address, but it values your opinions as being representative of others in your area.

RTI International, a non-profit research company, is conducting this survey for FDA. In the next few weeks, RTI will call your household to ask a few simple questions to determine if your household is eligible for the survey. RTI will also provide more information about the survey to help you decide whether or not you would like to participate.
