SDS Information Page


Item 1.A: Total Federal Funds Authorized for this Funding Period: This amount is automatically inputted based on Notice of Award.


Item 1.B: Total Amount of line 1A Disbursed: Enter the total amount of money drawn down from PMS and disbursed to students for this funding period.


Item 1.C: Unused Balance of Federal Funds: Line 1.A. minus Line 1.B. This amount is automatically calculated by the system.


Item 1.D: Check to indicate zero disbursements: Please select the check box if none of the amount indicated in Line 1.B. was disbursed


Item 2: Did your BHPR funded program have students of "Hispanic or Latino Ethnicity" during the reporting period? If the answer selected is “Yes”, provide the following data by race (Item 2.A – 2.F).


Number of Students Receiving Awards Who Did Not Graduate: Indicate the number of students receiving SDS funds that did not graduate during this reporting period for each race. DO NOT INCLUDE STUDENTS THAT GRADUATED DURING THIS REPORTING PERIOD. Count each student once.


Number of Graduates the Received Awards: Of the number of students entered by race, who have received awards, indicate the number of students who have graduated during the reporting period for each race. Count each student once.


Item 2.A-E: Refer to Ethnicity and Race definitions.

Item 2.F: Click the “here” button to navigate to the “More than One Race Combination for Hispanic or Latino Students” page. Select race combinations and enter counts for students and graduates who received SDS awards for the current discipline.

If you are unable to provide the counts or there are no students by race combinations, provide reason in the comment field at the bottom of the page. Once the comment is provided and upon “Save and continue”, the fields corresponding to Item 2.F In the SDS Information Page will be editable. Please enter the counts in these fields accordingly.

*Note: If there are no students by race combinations, please enter “0” in Item 2.F on SDS Information Page after providing, comments in “More than One Race Combination for Hispanic or Latino Students” page.

ETHNICITY:

  1. Hispanic or Latino: A person of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture or origin, regardless of race. The term, "Spanish origin," can be used in addition to "Hispanic or Latino."

  2. Not Hispanic or Latino: A person not having origins of Mexican, Puerto Rico, Cuban, Central or South American or other Spanish culture or origin, regardless of race.


RACE:

American Indian or Alaska Native: A person having origins in any of the original peoples of North, Central, or South America, and who maintains tribal affiliation or community attachment.

Asian: A person having origins in any of the original peoples of the Far East, Southeast Asia, or the Indian subcontinent including, for example, Cambodia, China, India, Japan, Korea, Malaysia, Pakistan, the Philippine Islands, Thailand, and Vietnam.

Under represented Asian: A person having origins in any of the original peoples of the Far East, Southeast Asia, or the Indian subcontinent other than Chinese, Filipino, Japanese, Korean, Asian Indian or Thai.

Black or African American: A person having origins in any of the black racial groups of Africa.

Native Hawaiian or Other Pacific Islander: A person having origins in any of the original peoples of Hawaii, Guam, Samoa, or other Pacific Islands.

White: A person having origins in any of the original peoples of Europe, the Middle East, or North Africa.

More Than One Race: A person identifying with more than one of the above racial categories.

Underrepresented Minorities (URMs):

  • All Hispanic/Latino are Underrepresented Minorities (URMs)

  • Only American Indian or Alaska Native, Asian underrepresented, Black or African American, and Native Hawaiian or Other Pacific Islander, Non-Hispanic/Non-Latino will be considered URMs

Item 4: For students that received SDS funds, enter total number of full-time students who have graduated in the specified academic year and are serving, or in residencies, in medically underserved communities during this reporting period.

Item 5: For students that received SDS funds, enter total number of full-time students who have graduated in the specified academic year and are practicing, or in residencies, in primary care during this reporting period. (Primary care is limited to allopathic medicine, osteopathic medicine, dentistry, nursing graduate and physician assistant.)

"Medically Underserved Communities" means any geographic area and/or population served by any of the following practice sites:

  • Community Health Centers (section 330)

  • Migrant Health Centers (section 329)

  • Health Care for the Homeless Grantees (section 340)

  • Public Housing Primary care Grantees (section 340A)

  • Rural Health Clinics, federally designated (section 1861(aa)(2) of the Social Security Act)

  • National Health Service Corps Sites, freestanding (section 333)

  • Indian Health Service Sites (Pub. L. 93-638 for tribal operated sites and Pub.L. 94-437 for IHS operated sites)

  • Federally Qualified Health Centers (section 1905(a) and (1) of the Social Security Act)

  • Primary Medical Care, Mental Health, and Dental Health Professional Shortage Areas (HPSAs)(designated under section 332)

  • State or Local Health Departments (regardless of sponsor - for example, local health departments who are funded by the State would qualify)

  • Ambulatory practice sites designated by State Governors as serving medically underserved communities

  • Practices or facilities in which 50 percent or more of the patients are uninsured or Medicaid recipients.
