Secondary educational institutions that desire to host a National Defense Cadet Corps Unit (NDCC) may apply by using a DA Form 3126-1. The DA Form 3126-1 documents the agreement and becomes a contract signed by the institution and the U.S. Government. This form provides information on the school's facilities and states specific conditions for the establishment of a NDCC unit. The data provided is used to determine which schools are to be selected for a unit.
