Justification for changes to Possession, Use, and Transfer of Select Agents and Toxins, OMB 0920-0576 after submission
CDC requests the following changes to Possession, Use, and Transfer of Select Agents and Toxins:
APHIS/CDC Form 1-added headers back to form because the original submission to OMB was missing headers for Sections 5 & 6B-6I.
APHIS/CDC Form 1-changed the header from “Middle Initial” to “DOJ Unique Identifier Number” since the DOJ number more accurately identifies the person instead of the middle initial. For example, in the past we had the individual put the middle initial to distinguish between 2 individuals (e.g., Mike “E.” Smith versus Mike “S.” Smith) and this was replaced with the “DOJ Unique Identifier Number.”
There is no change in burden or respondents as a result of this change.
These changes apply to IC #1 – Application for Registration. The new form has been included as an additional document with IC #1.
