A11: Added 2 sentences before the question. New wording:

C1: Added underlined sentences:

As I read the list, please tell me Yes or No whether you took the medicine. During this time period, did you take any of the following medications? READ CHOICES
Re: C1 - NOTE: In the above new wording, I broke it into 2 sentences to be more correct grammatically. CDC sent it to Battelle as: I will read you a list of medications as I read the list, please tell me Yes or No whether you took the medicine.
Molly approved my breaking it into 2 sentences in the English and Spanish hard copies, but it will need to be changed in the new CATIs, as well.
Appendix, pg 88: changed “about 15 coding lists” to “17 coding lists” to accurately reflect the actual number of coding lists used.

D39 – Deleted question about brand of cooking oil. Keep D39a about type of cooking oil.
J22 - Were you married around

A39 – Add “for this pregnancy” to end of question if Father is unknown, so it matches question parameters when father is known.
G1 – Instead of using “around the time you got pregnant” as a prompt, add it to the end of the question.
J16 – Change HC to match wording in CATI for TABs to make consistent with other TAB wording: Replace “Did any of this baby’s grandparents, uncles…” with “Did any of the grandparents, uncles…”
J18 –This question is about the person’s sex/gender, not relationship, so changed the wording to: ASK ABOUT SEX ONLY IF IT HAS NOT YET BEEN STATED. Is this person male or female?
E9 – In the response list, replace “Fairly Often” with “Somewhat Often”.

D32ii1 & ii2 – changed “with skin” and “without skin” to lower case, as they’re read to R. CATI has them correctly in lower case.
D34 – changed response wording to match tense of question.
D34 – Removed the last word “…you…” from the question to match the new response wording.
Changes to Appendix in HC:
Medication frequency responses (in all medication questions) - combine codes for 90 & 91 and change label to 90 = IV (any)
Time Periods: - took out reference to -3, -2, etc, as CATI no longer uses those designations.
Changes to Table of contents:
Updated the Table of Contents to reflect changes in pagination.
Miscellaneous formatting changes:
B75 – the response code numbers didn’t show up after widening the column next to it, so changed the margin so that the codes appear.
Removed several intentionally blank pages no longer needed, so pages will align differently and the total number of pages will be less.
D32mm – added a period after question number.
Spanish Version:
B78 and B88 are now in the correct order
Updated the appendix to match the English version
D32n: Revised translation should now say:
"Durazno o melocotón, chabacanos (o albaricoques), ciruela, o nectarina (1 fresco o 1/2 taza enlatado)"
