Summary of Changes
We have made the following changes to From SSA-3:
Deleted “the” in the first sentence on the front page;
Inserted “reverse” at the bottom of the front page.
As a result of a systematic review of forms by the SSA Office of General Counsel, we have revised/updated the Privacy Act Statement .
We will issue old stock until exhausted and then we will use the revised version.
