FIELDING INSTRUCTIONS FOR THE

USE OF THE COMMON INFORMATION COLLECTION INSTRUMENTS

Thank you for your assistance with fielding the common information collection instruments. Your ACTION partnership will summarize and synthesize the information from these instruments. The individual contributions and responses from your hospital will not be identifiable or shared beyond your ACTION partnership. The point of contact (POC) for your facility’s ACTION partnership is ___________________________________________(insert name and location).

When providing the instrument(s) to your colleagues, please share the HAI Project Overview with them. All information being collected from the various instruments will be handled CONFIDENTIALLY. Responses to the questions are important; however, your colleagues may leave blank any questions they do not wish to answer. If you have questions about any of the information collection instruments or the HAI project, please contact your ACTION POC.

As part of this project, the assessment program will collect and analyze the information from these instruments. They will also develop an HAI Project case study which will be shared publicly so that others may benefit from the lessons learned from this project.
