REACH US is a community intervention
program designed to eliminate health disparities in minority
communities. As part of the program evaluation, CDC will conduct
annual telephone surveys in the intervention communities. It is
estimated that 29 communities will participate in the survey and
900 completed interviews will be performed in each community each
year.
