This form is
approved for government-wide use. Any grant-making agency that
intends to use the survey under ED's approval must contact ED and
provide the necessary burden information. ED will then submit a
change worksheet (83-C) to OMB to account for the additional
burden. ED will follow-up annually with all participating agencies
to ensure that the burden hour calculation remains correct. Any
changes to burden hour calculation will be adjusted through a 83-C.
At this point in time, ED and grants.gov have identified this form
as a potential candidate for adoption as a government-wide form
under grants.gov. ED will continue to work with grants.gov to
determine if grants.gov can assume burden and OMB clearance
responsibilities for this form.
