Attachment A—CHOP IRB Exemption Determination

The IRB has completed the review of IRB # 6342, "Developing Best Practices to Encourage Non-Residential Father's Participation in the National Children's Study: Perspectives from both Unwed, Non-Residential Mothers and Fathers".
Because the IRB has determined this to be exempt, we will have no further involvement with this. Therefore, I have edited the consent form to remove reference to the IRB. I am attaching the edited and a clean version of the consent. Please use this version for all subject enrollment.
