assist ATSDR with obtaining information from customers regarding
their satisfaction with its Web site. This information will help
ATSDR ensure that customers find the information easy to access,
clear, informative and useful. The survey is only available in
electronic format via the ATSDR Web site. The survey is completely
voluntary and does not collect personal identifiers.

Records for the previous
approval (change request) indicate 415 approved burden hours
involving 4981 responses. These figures appear to be incorrect and
to more closely approximate 3-year totals rather than annualized
totals. The current reinstatement request is based on an annualized
estimate of 2000 total responses. Please note the following: 1)
Distribution of the 2000 estimated responses over the base survey
(WSUS) and 5 replicate product-specific surveys (TCCUS, PHSUS,
TPUS, TFUS, and TP-CDUS). 2) Separation of Affected Public into 2
ICs (one IC for Individuals/Households, 90% of total responses, and
one IC for the Private Sector, 10% of total responses). Both
segments of the Affected Public are assumed to use all available
User Satisfaction Surveys.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
