Below are ACF’s answers to the remaining OMB questions.

We have heard from our General Counsel regarding question one. In all instances that address
privacy, we are adding the caveat “private to the extent permitted by law.”
•
In addition, we have attached the final survey versions for OMB’s review. OMB had mentioned
that it would be helpful to get a track changes version. This is not available because the survey
has gone through so many round of revision that it would be impossible to track the levels of
change from one revision to the next. I have attached a report on the results of cognitive testing
which can inform those reviewing the survey regarding the changes that have taken place as a
result of cognitive testing. In addition, I have attached key milestone versions for comparison.
These include the following:
1. original first draft version from before cognitive interviews,
2. second draft version from after the first round of cognitive testing,
3. third draft from after the second round of cognitive testing, and
4. final draft version – this is the most important survey to note, since it is our current
and final version.
5. Household screener interview
