Revision to the collection instrument
The only revision requested is the removal of the Social Security Number block from the top line of the form. This in no way alters the purpose or method of collection of the data it merely provides an additional safe guard for protecting individuals’ personal information This form will be completed by realtors at the local level about 90 percent of the time. They do not have the need to know an individual’s Social Security Number in order to give an estimate of market value of real estate.
