Additional Burden Document

The National Evaluation of the Comprehensive Community Mental Health Services for Children and Their Families Program:  Phase V - (OMB No. 0930-0280) was approved on 10/16/06 through 10/31/09.  SAMHSA awarded grant funding for 5 additional grantees under this Program for FY 2006.  As with our other grantees, these 5 new grantee sites are required by their GFA to participate in the national evaluation.  NO changes have been made to the national evaluation instruments or data collection protocol (as described in the Supporting Statement that received final OMB approval on October 16, 2006).  Does SAMHSA have to submit a whole new OMB package or can I just do an 83-C form or equivalent for the additional hours to account for the newly-awarded grantee sites? The original burden table covering the 25 grantees funded in FY 2005 (for which OMB approval has been received) is below.  The second table below summarizes the additional annualized burden estimates that is needed for the 5 FY 2006-funded grantees.  Summary of Annualized Burden Estimates for 5 Years (Grantee Communities Funded in 2005)
