Abstract: The information collected by form SSA-6234 is used by SSA to determine if organizational representative payees (i.e. institutions, agencies) are using the payments they receive on behalf of beneficiaries for the beneficiaries' current maintenance and personal needs; to ensure that the payee continues to be concerned about the beneficiary's welfare; and to determine if the beneficiary is being charged a fee and if so, the amount of the fee. The respondents are organizational representative payees for beneficiaries receiving Social Security Benefits.
