Terms of Clearance: This survey is approved for 18 months. In reporting results of this survey the agency must cite the response rate (i.e., the completed responses as a percentage of all establishments contacted) and any trends in response rate (e.g., a lower response rate from smaller clients, from less frequent clients, etc. . .) In future submissions of this ICR, the agency must provide information about past response rates and document a plan to obtain a sufficient response rate (generally considered to be 80%). If the sufficient response rate cannot be obtained, the agency must provide a plan to identify sources of nonresponse bias and account for them in reporting.
