Abstract: The I3820 will collect information about the condition of a child who is applying for Social Security disability benefits. It is an alternative to using a paper SSA-3820. It will be used by an applicant for child's disability beneifts who prefers to file via the Internet. It will solicit the details of the child's condition, how the condition affects the child's day-to-day life, and information about medical treatment sources. The I3820 will collect information about the condiiton of a child who is applying for Social Security disability benefits.
