DASIS is conducted to address the
mandates of Section 505 of the Public Health Service Act for annual
information about public and private substance abuse treatment
providers and the clients they serve. Approval is requested for
identification of new treatment providers, update of information on
current identified providers, and a questionnaire used for the
annual survey of all identified providers. The annual UFDS survey
questionnaire is being slightly revised and a Mini-N-SSATS is being
added to obtain information from newly identified providers for the
Facility Treatment Locator. Aggregate client data are provided
