poor response rate from the last customer satisfaction survey
undertaken by ATSDR. OMB also notes that the supporting statement
identifies several actions and procedures that ATSDR will undertake
to improve response rate for this survey. Specifically, OMB notes
that ATSDR "will encourage greater participation by . . .
government customers by telephoning them if the health consultation
survey is not completed in two weeks."_The supporting statement
also notes that mailed questionnaires will include a "franked
return address flap".

This information collection allows
ATSDR to evaluate our customers' satisfaction regarding several
ATSDR public health products. These products will include public
health assessments, health consultations, and site-specific fact
sheets. Respondents will include community members living near
hazardous waste sites and state or federal employees working at or
involved in hazardous waste sites. ATSDR hopes to gain valuable
information on the usefulness and timeliness of its public health
products in order to make appropriate improvements and increase
customer satisfaction.
