SSA-831 is used by the Social Security Administration (SSA) to
document the State Disability Determination Services (SDDS)
decision about whether an individual who applies for disability
benefits is eligible for those benefits based on his or her alleged
disability. SSA also uses this form for program management and
evaluation. The respondents are SDDS employees who make disability
determinations for SSA.
