submit to OMB a plan for achieving an 25% response rate and
evaluating item and general non response bias. Any reimbursement
incentives must be approved by OMB prior to implementation.

The purpose of this data collection is
to evaluate the impact of an organ and tissue donation awareness
campaign on college campuses designed to increase the rate of
intentions to be organ and tissue donors, specifically, a College
Donor Awareness Project (CDAP).
On behalf of this Federal agency, I certify that
