statements mandated by the Paperwork Reduction Act of 1995. For the
public record, the AoA must submit to OMB the revised
forms/instructions.

Two questions for 55 States and
territories (and 18 tribes) to determine if SUAs systems are
working properly after the Y2K conversion and if there are any
elderly at risk in the State due to Y2K disruptions.
