Disability Request forms as soon as is practical. (3) In submitting
these forms to OMB for PRA clearance, ED will include in the
supporting materials a final copy of the *Corrective Action Plan*
developed in response to the OIG's June 7, 1999 report on loan
forgiveness. Also, the supporting materials will include ED's
answers to the OMB memo of June 24, 1999.

a loan discharge based on permanent total disability or a loan
deferment.
