This information
collection is approved with the following conditions: The Paperwork
Reduction Act statement must appear, and be easily located on all
internet surveys. No phone surveys are approved under this
clearance without additional review by OMB of the methodology. Mail
survey response rate must be raised, either by shortening the
survey or increasing the number of contacts with respondents. The
Paperwork Reduction Act statement must clearly inform the
respondents that the agency may not conduct or sponsor, and a
person is not required to respond to, a collection of information
unless it displays a currently valid OMB control number. Internet
survey and mail survey results cannot be aggregated, since the
sampling methods differ and the internet survey is not a random
sample.
