through 8/2000 under the following conditions: 1) no later than
2/98, PHS submits to OMB its plans for tracking the response rates
achieved by each of these surveys, evaluating nonresponse bias, and
addressing these and other issues that may impede the PHS' ability
to make statistical inferences from these customer satisfaction
data; 2) for each survey instrument, the PHS immediately
incorporates the new disclosure statements mandated by the
Paperwork Reduction Act of 1995. For the public record, the PHS
must submit to OMB the revised forms/ instructions; and 3) in
regard to the FDA Medical Bulletin survey, in question 2 FDA
reverses its five point responses so the "5" response reflects
"extremely useful" and FDA moves question 6 to the end of the
survey to minimize nonresponse and facilitate evaluation of the
characteristics of any nonresponse bias.

These voluntary customer surveys will
be used to implement E.O. 12862 within the public health service
programs. Most respondents will be members of the public or health
care professionals.
