This collection
is approved as amended on the following conditions. First, one-year
approval should provide ED enough time to significantly revise the
form to focus more on performance and outcome information for
individuals served by the program. As stated in OMB's memorandum of
1/17/96, OMB believes that the form should undergo a major overhaul
and that plans to make the necessary revisions should begin now
while ED awaits reauthorization. Second, ED agrees to note in the
collection package that an electronic version of the form is
available to grantees upon request.

These performance reports collect
information required of grantees receiving Federal funds under part
D of IDEA, required by Public Law 101-476 and 102-119. Training
data will be summarized in OSERS' annual report to Congress,
including data on special education and related service personnel,
as well as parents trained.
