As ED has
requested in its 9/7/95 memorandum to OMB, this form is extended
without change from the currently approved version through January
1996, solely for applications from new institutions, institutions
that have changed ownership or address, and institutions where
problems require additional data for gatekeeping purposes. ED is
reengineering this form and the related process of institutional
eligiblity, and will report on its results and recommended changes
as part of its next clearance request. ED shall include in this
report status updates on 1) modifying IPEDS so that IPEDS data can
be used to assess school performance in determining eligibility,
and 2) efforts to formalize an automated version of this form so
that instutions can submit electronically; ED's updates in these
areas shall provide appropriate timetables to achieve as rapid a
transition as possible, in light of the 6/16/95 memorandum included
as part of the current request.

postsecondary educational institutions participating in the student
financial assistance programs authorized by title IV of the Higher
Education Act of 1985, as amended (title IV, HEA
programs).
