meet the following conditions: -- ED shall revise its schedules to
ensure that this form can be used by States to satisfy their annual
application requirements, thus eliminating the need for a separate
requirement. If this is not feasible, then approval is granted for
collection of this information only on a biennial basis. --
Regardless of frequency, this form will be allowed to satisfy the
requirement for a biennial evaluation of the early intervention
component. The instructions shall be revised to reflect this,
adding any items required to be included in the biennial evaluation
under the statute and regulation (34 CFR 693.52).

State agencies use this performance
report to account for the yearly program performance under the
NEISP Program. The Department uses the information collected to
assess the accomplishment of the program goals and objectives and
to aid in program management and compliance assurance.
