rnformation is needed to monitor
compliance of DA&A beneficiaries with the requirements of P.L.
103-296. Information is used to determine eligibility for and
retention of Social Security and SSI benefits. The respondents are
individuals who are disabled based on DA&A, referral and
monitoring agencies and treatment facilities.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
