Abstract: FOR A BORROWER SEEKING REPAYMENT OF A DIRECT LOAN UNDER THE INCOME CONTINGENT REPAYMENT PLAN, THIS FORM WILL PROVIDE THE REQUIRED 5-YEAR CONSENT FOR THE INTERNAL REVENUE SERVICE TO DISCLOSE CERTAIN TAX INFORMATION TO THE DEPARTMENT OF EDUCATION FOR PURPOSES OF CALCULATING THE BORROWER'S MONTHLY REPAYMENT AMOUNT.
