THE INFORMATION IS NEEDED TO ENSURE
THAT SECTIONS 403(G)(1)(A) AND 402(I)(6) OF THE SOCIAL SECURITY ACT
ARE BEING EFFECTIVELY IMPLEMENTED IT AFFECTS 50 STATES, THE
DISTRICT OF COLUMBIA, AND THE TERRITORIES OF GUAM, PUERTO RICO, AND
THE VIRGIN ISLANDS.
