AND DISABILITY CLAIMS PAID TO LENDERS AND FOR COSTS INCURRED FOR
SUPPLEMENTAL PRECLAIMS ASSISTANC AN AGENCY MAY USE THE FORM TO MAKE
PAYMENTS FOR AMOUNTS DUE ED FOR COLLECTIONS ON DEFAULTED LOANS ON
WHICH REINSURANCE HAS BEEN PAID AND
