collection is approved through 8-92 under the following conditions:
SSA must add a part (b) to question number two, to determine how
many different days the caller unsuccessfully tried to get through
on the 800 line, if the caller responds that he/she tried on
subsequent days. SSA must also make it clear that question number
12 (asking for SSN) is completely voluntary.
