Abstract: THE FORMS ARE NEEDED FOR LENDERS TO MAKE APPLICATION TO THE HEAL INSURANCE PROGRAM, TO REPORT ACCURATELY AND TIMELY ON LOAN ACTIONS INCLUDING THE LENDER CURRENTLY HOLDING THE LOAN, AND TO ESTABLISH THE REPAYMENT STATUS OF BORROWERS. THE REPORTS ASSIST THE PHS IN PROTECTING ITS INTEREST IN THIS LOAN INSURANCE PROGRAM.
