THE BENEFIT OF A RECIPIENT OF SUPPLEMENTAL SECURITY INCOME (SSI)
PAYMENTS SHOULD BE MODIFIED OR DISCONTINUED BASED ON THE CHANGE
REPORTED. THE AFFECTED PUBLIC CONSISTS OF SSI RECIPIENTS WHO USE
THIS FORM TO REPORT CHANGES TO SSA.
