STATES TO SUBMIT REQUESTS DIRECTLY TO THE FEDERAL PARENT LOCATOR
SERVICE. THESE STATES USE THIS FORM TO OBTAIN ADDRESS INFORMATION
AND SOCIAL SECURITY NUMBERS OF ABSE PARENTS FOR THE PURPOSE OF
LOCATING ABSENT AND ENFORCING CHILD SUPPORT OBLIGATIONS. STATE
AGENCIES ARE THE AFFECTED PUBLIC.
