Abstract: FORM PROVIDES A UNIFORM METHOD FOR RECORDING INFORMATION OBTAINED DURI AN INITIAL SURVEY OF A CONTRACTOR FACILITY. THE PURPOSE OF THE SURVEY IS TO 1) DETERMINE ELIGIBILITY FOR A CONTRACTOR FOR ENTRY AND PARTICIPATION IN THE INDUSTRIAL SECURITY PROGRAM AND 2) DETERMINE ABILITY OF A FACILITY TO PHYSICALLY SAFEGUARD CLASSIFIED INFORMATION.
