ELIGIBLE PARTIES MUST APPLY FOR SUCH SERVICES THROUGH ED FORM 926, USE FEEDBACK FORMS (ED 926-1 & 926-2) ARE USED TO EVALUATE THE SERVICE PROVIDED BY THE LOAN PROGRAM. FEEDBACK FORMS REQUEST ATTENDANCE AND CIRCULATION DATA.
