Abstract: SECTION 216(H)(1)(A) OF THE SOCIAL SECURITY ACT REQUIRES THAT A CLAIMANT MUST BE VALIDLY MARRIED TO THE WAGE EARNER IN ORDER TO BE ENTITLED TO CERTAIN TYPES OF BENEFITS. THESE FORMS ARE USED RESPECTIVELY TO OBTAIN EVIDENCE FROM A PARTY TO A CHINESE CUSTOM MARRIAGE OR FROM A THIRD PERSON HAVING KNOWLEDGE OF A CHINESE CUSTOM MARRIAGE WHEN A DETERMINATION ON SUCH MARRIAGE IS MATERIAL TO ENTITLEMENT
