SECTION 1611(C)(1) OF THE SOCIAL
SECURITY ACT PROVIDES FOR INFORMATION REGARDING THE REDETERMINATION
PROCESS FOR SUPPLEMENTAL SECURITY INCOME (SSI) BENEFITS. THIS FORM
IS USED TO ELICIT MINIMAL INFORMATION FROM CAREFULLY SELECTED SSI
RECIPIENTS TO PERMIT DISTRICT OFFI PERSONNEL TO MAKE A DECISION ON
CONTINUING ELIGIBILITY OF RECIPIENTS OR THAT FURTHER DEVELOPMENT IS
NECESSARY.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
