SECTION 233(A) OF PUBLIC LAW 95-216
(INTERNATIONAL AGREEMENT WITH RESPECT TO SOCIAL SECURITY BENEFITS)
PROVIDES FOR INFORMATION REQUIRED BY PERSONS FILING FOR SOCIAL
SECURITY BENEFITS UNDER THE AGREEMENT WITH SWITZERLAND. THIS FORM
WILL BE USED TO OBTAIN SUFFICIENT INFORMATION TO ENABLE BOTH
SWITZERLAND AND THE U. S. TO DETERMINE WHETHER ENTITLEMENT TO
BENEFITS EXIST.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
