ACT EXCLUDES FROM WAGES CERTAIN PAYMENTS MADE UNDER AN EMPLOYER'S
PLAN OR SYSTEM. THIS QUESTIONNAIRE ELICITS INFORMATION TO DETERMINE
WHETHER THE EMPLOYER HAS SUCH A PLAN. THIS INFORMATION IS NECESSARY
TO DETERMINE THE AMOUNT OF SOCIAL SECURITY BENEFITS PAYABLE ON THE
INDIVIDUAL'S EARNINGS RECORD.
