Terms of Clearance: The sponsor has agreed to the following: The cover letter that is sent with the two forms will include the authorized voluntary statement. Language will be added to allow the sstates to submit computer printouts if all of the data elements on the 1269 are captured. Change all references from OE to ED. The instructions and the form for the 1269 and 1269-1 will show the OMB No, expiration date, the form number and edition date. Minor changes will be made to the 1269 instructions for itemss 3, 4, 5 and 6. If the RFP for this data collection is contracted, the states will be provided with complete preprinted information to update (add/delete) the 1269.
