THE NATIONAL DEATH INDEX IS BEING
DESIGNED TO PROVIDE RESEARCHERS WITH A CENTRAL LOCATION FOR
DETERMINING WHICH PERSONS IN THEIR STUDIES MIGH HAVE DIED AND TO
DIRECT RESEARCHERS TO THE APPROPRIATE STATES WHERE MORE DETAILED
DEATH RECORD INFORMATION CAN BE OBTAINED FOR THE PERSONS IN
QUESTION. AN APPLICATION FORM AND FOUR RELATED FORMS ARE NEEDED TO
ADMINISTER THIS NEW SERVICE.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
