SECURITY ACT PROVIDES FOR CERTIFICATION OF PAYMENTS UNDER TITLE XVI
TO A PERSON, OTHER THAN THE RECIPIENT, WHO IS INTERESTED IN OR
CONCERNED WITH THE WELFARE OF SUCH INDIVIDUAL. THIS FORM IS USED IN
SUPPLY INFORMATION ABOUT THE USE OF BENEFITS AND TO DETERMINE
WHETHER TO CONTINUE MAKING PAYMENTS TO THE REPRESENTATIVE
PAYEE.
