THE PRINICIPAL PURPOSE FOR COLLECTING
THE INFORMATION REQUESTED ON THE PHS-50 IS TO DETERMINE THE
APPLICANT'S ELIGIBILITY FOR COMMISSIONING IN THE PUBLIC HEALTH
SERVICE COMMISSIONED CORPS. IF THE APPLICANT IS APPOINTED, THE
INFORMATION COLLECTED WILL BE USED FOR SUBSEQUENT PERSONNEL ACTIONS
SUCH AS TRANSFER, PROMOTION, AND IN DETERMINING ELIGIBILITY FOR
BENEFITS. IF NOT APPOINTED, THE RECORDS ARE RETAINED FOR TWO YEARS
AND THAN DESTROYED
