Abstract: SECTION 205(C)(4) OF THE SOCIAL SECURITY ACT PROVIDES FOR COLLECTION OF INFORMATION REQUIRED TO CORRECT THE EARNINGS RECORD OF A WAGE EARNER. THE FORM IS USED BY DISTRICT OFFICES TO DETERMINE THE PROPER AMOUNT OF EARNINGS TO BE CREDITED TO THE WAGES EARNER'S ACCOUNT.
